To ensure students are using the most recent version of a DGS form, please download forms directly from the DGS website www.asu.edu/graduate/forms/index.html.

Please note the current graduate Program of Study and Course Changes to a Graduate Program of Study forms are available on the Registrar's website: http://www.asu.edu/registrar/forms/regforms.html#4.

iPOS Info:

The new web-based official program of study has been online since February 14, 2006. As of March 1, 2006, all students who do not have a paper copy of the Program of Study already on file will need to complete the Interactive Program of Study, regardless of whether or not you are in the process of obtaining signatures. The University will no longer accept paper programs of study after February 28, 2006.
The new process is as follows:
Point your web browser to http://my.asu.edu/.
  1. Enter your ASURITE ID and password and click the Log-in button.
  2. Upon successfully logging in, click on the "Graduate Studies" tab.
  3. Click on the Interactive Program of Study
  4. Follow the online instructions, being certain to Save and Continue after entering the information on each page.
  5. Once you have completed the iPos, click "Submit," you will then be routed to a page where you can print off the committee approval page and the course listing page. You will need your committee's signature for the department to submit the final electronic approval to the Graduation Office.
  6. Turn in the committee signature page and course list to Debbie Olson for department graduate director approval who will then submit your iPos electronically for approval.

Master's in Passing applications and program of study must still be done on the paper forms for now.

Keep in mind that DGS must have an official transcript from all institutions you plan to transfer coursework from (particularly for PhD students). Please make sure you have requested all official transcripts be sent to DGS or you will not be able to list that coursework on the program of study.
